Program Enrollment
Project 2 (P2) is one of ACHIEVE GREATER's three related research projects. P2 aims to prevent heart failure in Black adults with high blood pressure. In P2, some patients work with a Community Health Worker (CHW) and a pharmacist to get personalized blood pressure care.

Will I be compensated?
During the course of the 2-year study, you can receive up to $50 in compensation for your time.

What are the benefits?
If chosen for intervention, you will be paired with a Community Health Worker (CHW) and a care provider to help with hypertension management. You may also receive a digital scale and blood pressure cuff at no cost to you.
Will my information be safe?
All information collected about you will be kept confidential. While your medical records may be reviewed so the clinical team can monitor your health and blood pressure, you will only be identified in the research records by a code name or number. We will not publish any identifying information about you.

How do I qualify?
To become a part of any ACHIEVE GREATER programs, potential participants are asked to visit a Wayne Health Mobile Unit for a blood pressure reading and health screening:

Self-identified African/black Americans

Living in the metro Detroit area

18 or older
-
At risk of or currently have hypertension
